Position Summary:

  • Establishes collaborative working relationships with both physicians and health care teams; within the hospital as well as in the community, and with consumers and family members to initiate, implement and monitor community treatment orders
  • Facilitates rights advice and acts as a resource for the health care team in preparing for Consent and Capacity Board hearings
  • Completes all documentation as required by legislation, MOHLTC and Trillium Health Partners

Key Qualifications:

  • Professional qualification in a health related discipline
  • Minimum of 3 years of postgraduate experience in Mental Health; preferably in the community and hospital setting
  • Thorough knowledge of the Ontario Mental Health Act and Health Care Consent Act
  • Excellent communication and organizational skills
  • Excellent collaboration skills with consumers, families and other healthcare professionals
  • Exceptional problem-solving and decision-making skills
  • Demonstrated ability to work independently and as a member of an interdisciplinary team
  • Knowledge of community resources
  • Valid driver's license; relevant insurance and use of own vehicle
  • Ability to speak a second language an asset
  • Satisfactory performance and attendance required
  • Proficiency with computer skills
