Base Rate: $96,200

Job Rate (Experienced hire): $120,250

Max Rate (Maximum salary potential, achieved by progressing through this band): $144,350

Job Description:

Reporting to the Director – Professional Practice, the Manager, Professional Practice, in partnership with clinical programs, will provide leadership, coordination, mentorship and support to drive practice excellence and professional practice improvements across Trillium Health Partners. The successful candidate will lead a team of practice consultants, will be committed to patient-centered design; and has a passion for leading evidence-based practice transformation within an interprofessional setting.

 

We are seeking a dynamic candidate who has a demonstrated history of achieving results; leading complex change initiatives and who is able to think critically and strategically in a fast-paced environment. Further, the successful candidate consistently demonstrates our values of compassion, excellence and courage when leading the delivery of an exceptional patient and staff experience.

 

Responsibilities:

  • Champion a best practice approach across the organization through development, implementation, and evaluation of practice standards across clinical programs.
  • Lead the development and implementation of practice initiatives focused on supporting excellence in best clinical practices and serve as a key point of contact and stakeholder for organizational professional practice initiatives.
  • Support commitment to THP’s Best Practice Spotlight Organization (BPSO) designation through Best Practice Guideline (BPG) sustainment and evaluation across the organization.
  • Provide practice support for establishment/sustainment of unit and corporate level practice councils.
  • Provide practice support for assessment and evaluation of current state practice environments through the practice review process, highlighting practice findings, strengths, opportunities, and recommendation generation.
  • Provide mentorship, coordination and support to clinical educator and professional practice leader groups across all sites.
  • Accelerate standardized practice through representation on various corporate and program committees and advice on revisions to key clinical documents, such as policies & procedures.
  • Partner closely with enabling services such as Quality and Clinical Systems & Informatics to facilitate alignment and coordination of practice activities and metrics to support practice evaluation and planning.
  • Assist in the selection and analysis of practice-related performance indicators to monitor organizational, program, and/or unit level performance to ensure alignment with strategic direction.
  • Work collaboratively with all staff and physicians to facilitate consistent interprofessional practice leading to the delivery of quality patient focused care.
  • Maintain links with Professional Colleges and Associations
  • Coordinate Human Resources and budgetary planning processes to ensure leadership and financial accountability

Qualifications:

  • Regulated Health Professional in good standing with their college is required
  • Previous experience in healthcare industry leadership (5+ Years) required;
  • A master’s degree in a clinical discipline, or in health;
  • Demonstrated professional practice leadership;
  • Demonstrated ability to coach, mentor and provide feedback;
  • Knowledge of Patient and Family-Centred Care principles is required;
  • Knowledge of applicable legislation, college regulations, standards and scope of practice;
  • Effective interpersonal and communication skills combined with the ability to work collaboratively within and across professions;
  • Previous experience in quality improvement is preferred;
  • Superior facilitation and change management skills;
  • Superior written and verbal skills;
  • Superior organizational and time-management skills;
  • Excellent decision-making, problem solving, and conflict resolution skills;
  • Excellent work ethic and positive attitude that demonstrates THP values
