Unit Summary:
THP is a Regional Cardiac Program providing services and care in collaboration with our Regional Partners. 2C Cardiology is a 50-bed inpatient, acute care unit. We support a wide variety of complex cardiac patients, while working closely with Cardiac Cath Lab, Cardiac Short Stay and Coronary Care Unit. We primarily focus on medical management and minimally invasive procedures to care for our patients in their unique care needs. We also support off-service patients with telemetry needs. 3A CSU is a Regional Integrated Stroke Unit that provides acute and rehab care to stroke patients. Our staff provide lifesaving treatment (TPA and EVT (mechanical clot retrieval)) for those afflicted with stroke. 3A is a 52 bed Unit (32 acute beds and 20 rehab beds). As a Regional Stroke Centre we receive stroke bypass from the following hospitals: CVH, Etobicoke, Brampton, Oakville, Georgetown, Milton, Orangeville, Humber and St. Joseph.

At Trillium Health Partners the Mental Health programs provide psychiatric assessment, short-term treatment and stabilization for adults and seniors. The program areas are staffed by a multidisciplinary team that includes psychiatrists, nurses, social workers, occupational therapists, and pharmacists. Position Summary:
The Disability Case Manager/WSIB Consultant is a member of the People Safety and Support team and is responsible for the proactive and expert management of WSIB, STD and LTD claims. This position collaborates with managers and other key workplace parties in injury and disability prevention, early intervention and return to work support. This position falls under our ‘Flex First’ remote hybrid working model, which allows for the flexibility of the right mix of in-office and remote work that suits the needs of the portfolio.
Responsibilities:
- Responsible for initiating, engaging, and maintaining contact with the injured/absent employees on a regular basis.
- Provide support to employee’s manager/supervisor and HR regarding claims management, disability, accommodation and the Attendance Awareness and Support Program.
- Interpret and educate managers on a variety of regulatory and compliance requirements to ensure that case management practices are applied in accordance with legislation, regulation and THP policies.
- Develop effective and sustainable return to work plans and conduct effective return to work meetings.
- For WSIB claims, ensure compliance with the Workplace Safety and Insurance Act. This involves managing all elements of the claims process including forms filing, incident investigation, return to work program development, work rehabilitation program coordination, compensation awards/claims review, and objection letter preparation.
- Where applicable, represent THP at all WSIB/WSIAT hearings and appeals.
- Maintain and respect confidential information; objectively and accurately document case history, strategy and case management plan.
- Collaborate and liaise with other members of People Safety and Support team.
- Collaborate with the employees, managers, unions, human resources and relevant health care professionals to implement return to work and/or modified work programs and support a variety of prevention and Healthy Workplace programs and initiatives.
- In collaboration with all managers and human resources business partners, regularly monitor and adjust the progress of all return to work cases to resolve emerging issues.
- Make referrals to appropriate resource such as EFAP provider, Occupational Health Physician, or Ergonomic Consultant, as required.
- Act as a liaison to all internal and external partners.
- Monitor proposed or pending medical guideline changes, WSIA legislation or WSIB policy changes and prepares THP responses or positions when warranted.
- Accountable for regularly gathering, maintaining and reporting statistics regarding for WSIB, STD and LTD, and be able to proactively identify trends in order to prevent reoccurrences.
- Able to effectively and collaboratively resolve conflict.
- Effectively manage a fluctuating case load.
- Must be able to work at all sites.
- Other duties as assigned.
Key Qualifications:
- At least 3 years of experience in disability and WSIB claims management.
- Post-secondary degree in the health sector.
- Must meet the eligibility requirements and become certified with the Canadian Society of Professionals in Disability Management as a Certified Disability Management Professional (CDMP) or Certified Return to Work Coordinator (CRTWC) within 2 years or be a CDMP or CRTWC in good standing.
- Demonstrated expertise and experience with return to work planning, HOODIP administration, long term disability contracts, collective agreements, and attendance and accommodation programs in a large and diverse organizational environment.
- Strong decision making and problem solving skills. Ability to create and monitor progressive and successful individualized employee modified work programs.
- Excellent interpersonal and communication skills to work effectively with diverse roles within THP, as well as outside agencies.
- Working knowledge of relevant legislation (e.g., Workplace Safety and Insurance Act, Ontario Human Rights Code, Occupational Health and Safety Act, and Accessibility for Ontarians with Disabilities Act).
- Must have analytical skills to maintain and interpret statistical data in order to recommend policies and procedures with respect to WSIB claims, documentation, and appeals.
- Self-confidence to provide mediation and dispute-resolution services.
- Excellent communication, organization, and presentation skills.
- Must be proficient in MS Office Suite.

Location:

Reporting to the Manager, Clinical Resource Team (CRT) the selected candidates of the CRT will provide clinical expertise and act as a team member while they work across a variety of units and sites. Being highly visible and an essential component of the interdisciplinary team, the CRT staff will have the opportunity to build relationships, while acquiring additional clinical experience. Qualifications:
- Graduate of a recognized School of Nursing
- Baccalaureate degree (Preferred)
- Registered in good standing with the College of Nurses of Ontario (CNO) or currently meet the eligibility criteria to register in good standing in the Temporary Class or General Class with the CNO.
- Current B.L.S.
- A minimum two years medicine experience
- Reliable and flexible individual who is willing to work collaboratively within an interdisciplinary team environment
- Must have effective interpersonal, communication, organizational and problem solving skills
- All shift availability
- Evidence of ongoing professional development
- Patient assessment course (Preferred)
- Satisfactory attendance and performance record required
