Position Summary:

Clinical Coordinator functions as the conduit between patients, therapists, physicians, clerical staff and THP Leadership. It requires leadership of quality assurance activities, administrative responsibilities, and collaboration with partners and stakeholders. This requires a clinical professional to advocate for both patients and staff to ensure a solid partnership. The rehabilitation process for hand therapy patients is complex – ranging from post-op care within 48 hours to conservative non-operative care, both of which are based on specific treatment protocols. Unlike other outpatient therapy services, therapists working in hand therapy are frequently required to adjust treatment plans with every visit as the status of healing are acute. Due to this dynamic and fast-paced environment, it is necessary to have a clinical professional who is actively engaged in practice while providing leadership to ensure operations are aligned with THP goals of Quality, Access and Sustainability. 

 

Key Responsibilities:

The Clinical Co-ordinator:

  • Works closely with the program manager to ensure effective and efficient program operations.
  • Tracks and monitors key performance indicators and ensures the program adheres to them.
  • Reviews reports and provides coaching and feedback to the interprofessional team.
  • Acts as a lead contact and liaison with internal and external stakeholders.
  • Summarizes complex assessment findings and facilitates case reviews or audits.
  • Is a resource for the team on clinical matters,supports ongoing education and knowledge translation while ensuring Best Practices based on ASHT Guidelines and in consultation with Plastic Surgeons.
  • Performs clinical assessment and treatment work within professional scope as required

 

Key Qualifications:  

  •  Regulated Health Professional who is registered with relevant regulatory body (e.g. College of Occupational Therapist of Ontario) – required.
  • Membership with relevant Association – preferred.
  • Minimum 3 years relevant experience in assessing and treating patients with hand injuries/conditions.
  • Demonstrates leadership skills, fostering a positive team culture, promoting equity, diversity and inclusion.
  • Excellent written and verbal communication skills.
  • Experience providing services with a strong quality improvement focus.
  • Customer and client centered approach to delivering excellent service, clinically and administratively.
  • Evidence of continuing education, professional and personal development – required.
  • Reliable, flexible, creative, and self-directed professional; able to work effectively and collaboratively with an interdisciplinary team.
  • Exercises initiative and good judgment with ability to multi-task.
  • Strong ability to successfully coach, develop and mentor allied health professionals and physicians to improve program quality.
  • Demonstrates excellent interpersonal, time management and organizational skills.
  • Liaise with community health care professionals.
  • Participation in the collection and analysis of clinical outcome measures.
  • Coach, mentor and develop other health professionals within the program.
  • Work as part of an interprofessional team model, providing evidence based, outcome oriented care and recommendations.
  • Participates in continuous quality improvement and evaluation.
  • Contributes to the overall vision/mandate of the Program.
