We strive to safeguard the confidentiality, integrity, and availability of our hospital and patients’ information.   As an integral member of the Information Services leadership team, the Manager of Information Privacy is accountable to the Director of Health Information Management and Information Privacy and Security. The Manager will lead all activities related to the development, implementation and operation of Trillium Health Partners (THP) Privacy and Freedom of Information (FOI) programs.   The Manager is recognized as the organizational leader in the Information Privacy domain and will identify risks and organizational priorities which require the allocation of human and financial resources to successfully mitigate and implement. The Manager will lead a team of subject matter experts who work with THP stakeholders to support their business goals while ensuring alignment with the organization’s information privacy strategies.   The Manager is responsible for attracting, retaining and leading high quality privacy talent to ensure the area(s) they are responsible for are preforming as required, are fulfilling business requirements, are identifying and addressing gaps in products and services within their domain and are to engage other teams in this effort as required.   In doing so, the Manager is responsible for human resource administration and for balancing the department’s fiscal operating budget.   As a key leader in this role, you will liaise with internal stakeholders and healthcare disciplines on identifying and implementing the corporate privacy strategic vision. The role will also have a dotted line reporting relationship to the VP of Information Services and Chief Information and Privacy Officer.   Leadership Responsibilities - Identify and report on information privacy risks, threats, vulnerabilities and breaches and make recommendations on remediation opportunities to manage risks. - Implement and maintain privacy policies, procedures and controls in coordination with the Chief Information and Privacy Officer (CIPO) oversight committees and legal counsel for the collection, use and disclosure of personal health information and for ensuring alignment with the Ontario Personal Health Information Protection Act (PHIPA) and other applicable provincial legislation. - Contribute to the development of Information Privacy strategic plan and roadmap. - Implementation of the Information Privacy strategic and operating plans. - Perform ongoing privacy compliance monitoring and reporting to ensure adherence to organization policy and provincial and federal legislation (e.g. PHIPA). - Lead or commission forensic analysis on privacy incidents and sensitive investigations into employee conduct and misuse of confidential and personal health information. - Work with the key stakeholders to ensure the organization has and maintains appropriate privacy and confidentiality consent forms, information notices and awareness materials reflecting the THP’s practices and requirements. - Draft and review information sharing agreements (“Data Sharing Agreements”) with partner organizations. - Review various types of legal agreements (leases, purchases of software, services, etc.) for adequacy of privacy measures, and provide recommendations. - Develop materials and promote activities to foster information privacy awareness across the organization. - Organize the delivery of privacy training and orientation for employees, volunteers, medical and professional staff. - Participate in the development, implementation and on-going compliance monitoring of agreements to ensure privacy concerns, requirements and responsibilities are addressed. - Establish and maintain a mechanism to track access to protected health information and report on such activities. - Establish and administer a process for receiving, documenting, tracking, investigating and taking action on all complaints concerning privacy incidents. - Ensure all system-related information security plans align with privacy practices. - Maintain current knowledge of applicable provincial and federal privacy legislations (e.g. PHIPA, PIPEDA, FIPPA, etc.). - Manage the teams’ workload, assign and prioritize work based on assessment of risk to the organization. - Implement information privacy best practice procedures. - Liaise with The Ontario Information Privacy Commissioners office. - Ensure that projects, programs and other activities in IS are implemented with proper consideration given to information privacy. - Determine minimum privacy requirements for applications and systems based on policy, data sensitivity, exposure, and other factors. - Maintain current knowledge of privacy industry trends and technologies. - Ensure that all documentation and materials are regularly reviewed and up to date. - Vendor relationship management. - Provide organizational strategy and decision-making relating to FOI requests. - Develop and maintain the Freedom of Information program and ensure the hospital is in compliance with the Freedom of Information and Protection of Privacy Act (FIPPA) at all times. - Report and manage public relations risks introduced through FOI requests. - Provide advice and recommendations to the President and CEO (the hospital’s “head” under FIPPA) relating to the disposition of FOI requests. - Make decisions on how to process FOI requests, charge fees, provide time extensions, and redact or disclose information. Qualifications - Successful candidates will have demonstrated extensive experience in information privacy, including expert knowledge of the Personal Health Information Protection Act (PHIPA) and other government/industry privacy regulations. - The ideal candidates will, at minimum, have 5 years of work experience in information privacy working in a regulated industry, preferably health care. - A university degree or college diploma is required. In this role the Transitional Care Admissions Coordinator will work collaboratively with Discharge Planning Leadership and Post-Acute Program Leadership as well as other key internal stakeholders across the organization to optimize patient flow in and through these services by ensuring timely and strategic access. These efforts will be aimed to facilitate quality patient outcomes while meeting the growing demand of these services through the development of standard and transparent referral principles and practices within THP and to external providers. The Transitional Care Admissions Coordinator will further use their clinical knowledge and expertise to assess and work with hospital clinical teams to optimize the timing and accuracy of the referral process, helping to facilitate seamless transitions and hand offs between clinical providers. To this end they will serve as a content expert in Transitional Care spaces and a central point of contact for this program of services. The NP will lead case management for the clients of this program.   The NP will: conduct comprehensive health assessments; diagnose; provide treatment (including diagnostic, pharmacological and non-pharmacological therapies and counseling); develop, implement and document a plan of care in collaboration with family physicians, endocrinologists, and inter-professional team; provide education; refer to / liaise with other community services; and, provide follow-ups as appropriate. A strong primary care background coupled with experience in chronic diseases such as; mental health, COPD, cardiovascular and renal disease are necessary to support CCDC clients.
