Job Description:
Under the leadership of the Manager, Discharge Planning, the Social Worker will work within a fast paced environment as a collaborative team member of an interdisciplinary team. The primary goal of Discharge Planning services is to support and enable patients and families to make timely transitions through the hospital system. The Social Worker does all of this in an effort with the goal of providing an exceptional patient and family experience. The Social Worker understands the Flow and Capacity challenges of a hospital system and works to achieve that the patients are in the right place, at the right time, receiving the right care.
In addition to being one of the key points of contact for discharge planning to formulate, facilitate and coordinate discharge plans focused on the needs and goals of the patient and their family system they will also be responsible for:
- Employing critical thinking and creative problem solving to facilitate all discharge planning, including complex discharge planning situations;
- Providing in-depth discharged focused assessments, collaboratively creating the discharge plan with the team, the patient and their family and providing support, education and guidance through the implementation of the discharge plan;
- Facilitating a timely transition to the community; identifying safety gaps/challenges and addressing all barriers using most appropriate resources in partnership with internal and external partners;
- Assessing risk and need for psychosocial intervention;
- Providing psychosocial assessments to identify the need for and subsequent provision of clinical social work services and/or referral to Social Work.
Qualifications:
- Masters degree in Social Work (MSW) required
- Current registration and membership with Regulated College;
- 2-5 years clinical experience in an acute care hospital setting;
- Experience in Discharge planning is essential;
- Demonstrated ability to conduct psychosocial assessments and counselling and/or demonstrated ability to assess risk and need for referral to Social Work for psychosocial assessment and intervention;
- Demonstrated excellence in interview skills and ability;
- Current knowledge of community resources and initiative to proactively search for new resources;
- Experience with legislation that governs Public Hospitals (Public Hospitals Act, Substitute Decisions Act, Consent to Treatment Act, etc.);
- Extensive knowledge on how to effectively serve an increasingly aging and diverse population with complex medical, functional, cognitive and psychosocial needs;
- Comprehensive knowledge of the impact of social determinants of health on discharge planning, hospital LOS, Patient Flow and hospital system capacity;
- Expert communication, collaboration, organizational and problem solving skills;
- Ability to work independently as well as with an interdisciplinary team;
- Demonstrated ability to set priorities;
- Skilled working with a broad range of client groups and their significant supports.

Position Summary:
- Responsible to supervise, direct, train, schedule, evaluate, and coach front-line food service employees who are responsible for receiving, storing, preparing, portioning, serving, delivering and picking up patient food and supplies, and who perform food service ware-washing and cleaning duties. Follows CUPE collective agreement provisions.
- Conducts audits; tours food services and patient care units; and supervises staff to ensure departmental standards are met.
- Troubleshoots and coordinates resolutions to operational challenges such as equipment breakdowns, short staffing, and patient concerns.
- Prepares documents such as daily staff rosters, payroll timecards, shift reports, staff schedule edits, audit results, work orders, incident reports, etc.

Key Qualifications:

Trillium Health Partners is currently in the planning design stages of a long term infrastructure plan announced by the Ontario government which has dedicated significant funding to support the construction of new hospital projects across Ontario. Position Overview:
Trillium Health Partners is seeking an experienced Project Coordinator to join its Capital Planning and Redevelopment portfolio. Reporting to the Manager, Planning and Design (M-Site), the Project Coordinator will be responsible for supporting the Trillium Healthworks Project HP’s Broader Redevelopment Project (BRP) at M site, known as the Peter Gilgan Mississauga Hospital and will be expected to work both as part of a broader team as well as independently in coordinating certain aspects of projects as directed by the Manager, and Project Leads and Project Managers.
M Site project is a full replacement of the existing Mississauga Hospital, delivered under the Design, Build, Finance, Maintain (DBFM) model in partnership with the Ministry of Health (MOH) and Infrastructure Ontario (IO). The new build will accommodate acute beds and a full complement of clinical and non-clinical support programs.
Responsibilities:
• Support the delivery of THP’s Broader Redevelopment Project (BRP) at M-Site using IO’s DBFM model under the direction of the Manager, and the Project Leads, and Project Managers.
• Coordinate with various project stakeholders, both internal and external, to ensure associated project activities are identified and addressed accordingly.
• Develop and maintain project monitoring and tracking tools to facilitate decision making process and ensure successful completion of deliverables.
• Responsible for overall project document control and file management.
• Coordinate user group engagement plan and assist in preparation and distribution of meeting materials for all User Group Meetings, Workshops and ad-hoc project meetings (agenda, minutes, pre-circulation materials).
• Assist in Contract Administration functions such as the coordination of Request of Information (RFI) activities between THP, IO, the Development Consortium and the Planning, Design and Conformance (PDC) consultant.
• Coordinate the review process for Key Milestone Submittals and Variations, which include the distribution and collation of documents and information between internal and external stakeholders.
• Contribute in the development and review of project output specifications and drawings.
• Monitor the progress and quality of work being performed by various project stakeholders.

Skills & Qualifications

• Demonstrated skill with respect to project coordination, organization, and implementation of large complex redevelopment projects involving the engagement of multiple stakeholders.

The In-Patient Medicine Program at THP consists of a total of more than 350 beds across the CVH & MH and is staffed by large interprofessional teams who support the care of complex medicine patients within the Mississauga-Halton LHIN. Position Summary:

Job Description:
- Provision of nursing care for patients within an interprofessional environment, including assessment, treatment, teaching and education

Trillium Health Partners is currently in the planning design stages of a long term infrastructure plan announced by the Ontario government which has dedicated significant funding to support the construction of new hospital projects across Ontario. As part of this plan, THP is growing the multi-disciplinary team that will lead planning the design for the redevelopment and expansion of both the Mississauga Hospital and Queensway Health Centre sites. With constant inputs on design and planning considerations from all the respective expertise on the team, our goal is to work together to ultimately produce a holistic and inclusive design.
Position Overview:
Trillium Health Partners is seeking an experienced Project Manager to join its Capital Planning and Redevelopment portfolio. Reporting to the Manager, Planning and Design (M-Site), the Project Manager will be responsible for supporting the Trillium Healthworks THP’s Broader Redevelopment Project (BRP) at M-Site known as The Peter Gilgan Mississauga Hospital. Leveraging your experience in planning and design of health capital projects, the Project Manager will function as a key point of contact for the Development Consortium team, the Planning, Design and Compliance (PDC) Consultant, the internal project team, and THP internal stakeholders to advocate for quality design that supports THP’s Strategic Plan and Mission.
M Site project is a full replacement of the existing Mississauga Hospital, delivered under the Design, Build, Finance, Maintain (DBFM) model in partnership with the Ministry of Health (MOH) and Infrastructure Ontario (IO). The new build will accommodate acute beds and a full complement of clinical and non-clinical support programs.
Responsibilities
• Lead contact between the internal Master Plan team and external consultants including the Development Consortium (DevCo) team, the Planning, Design and Compliance (PDC)’s team (architectural, interior clinical planning team), Functional Programming Consultant, and other associated consultants for all items related to the development of the Project Specific Output Specification (PSOS);
• Manage the planning and design of wholesome building elements associated with Project Specific Output Specification (PSOS) including, the interior fit-out of the clinical and non-clinical programs, and the technical coordination of Structural, Mechanical, Electrical components, and other specialty topics such as wayfinding, accessibility and indigenous, to ensure alignment with overall Master Plan strategic priorities and Ministry of Health (MOH) directives;
• Lead for Departmental User Groups and Technical Meeting Coordination, which includes acting as the main THP project representative at user engagement sessions and speciality workshopsreview all associated meeting documentation (agenda, pre-circulation materials, minutes), manage homework action items and follow-ups, and facilitate any user change requests (if required);
• LeadLead the review process of key Milestone Submittal packages which includes drawings and specifications by departmental user group and technical working groups, overall review process and ensuringe alignment between multiple disciplines and priorities;
• Lead quality control and assurance activities to facilitate overall coordination and alignment during the PSOS development;
• Support the progression of the project through the MOH’s capital planning process;
• Support the process under IO’s DBFM delivery procurement model;
• Coordinate plans with other agencies having jurisdiction, including but not limited to the City of Toronto and IO;
• Contribute project budget development activities and risk management;
• Contribute to the development of master schedule related to all planning and design milestones and deliverables;
• Engage and communicate with key stakeholders on an ongoing basis to build trusted relationships to help advance key initiatives
• Other duties assigned, as required to meet timelines and deliverables.
Skills & Qualifications
• An undergraduate degree in architecture, engineering, interior design or a health care related field or an equivalent combination of education and experience will be considered.
• Minimum of 5 years project management and coordination experience working in a hospital setting or with a hospital as a client, ideally delivering a DBFM project.
• Minimum of 3 years experience working with the Ontario Ministry of Health’s capital planning process and/or Infrastructure Ontario’s DBFM model.
• Extensive knowledge in the planning, design and specification development of health capital infrastructure and operations, including international trends and best practice.
• Detailed understanding of the health capital planning environment in Ontario, including experience working with Ministry of Health, LHINs, Infrastructure Ontario, municipalities, and/or other agencies having jurisdiction.
• Demonstrated knowledge of building technical codes and standards, applicable to health care environments.
• Ability to interpret drawings, reports and studies of all design disciplines (architectural, electrical, mechanical and structural).
• Skilled at managing multiple stakeholders of different levels and disciplines, including at the executive level.
• Excellent written and verbal communication and presentation skills.
• Superior analytical, problem solving, negotiation and conflict resolution skills.

Preferred Qualifications:
- Baccalaureate degree in Nursing preferred
- Minimum of two years acute care medical/surgical experience
