The patient population includes patients with responsive Behaviors, Hemodialysis, Palliative, Convalescent, and complex medical needs. We are a collaborative group of healthcare professionals who strive for excellence in providingcompassionate, patient-centered care.We strive to reintegrate patients who no longer require acute medical care back into the community. Our Discharge planning team works collaboratively with patients and families to facilitate a smooth transition to a community setting. Our specialized team will support patients care plan goals while providing more social engagement and recreation therapyleading to enhanced wellbeing and quality of life for our patients.     Role Summary:    The successful candidate will be a highly motivated, agile and adaptable professional, with advanced clinical and critical thinking skills.  As an integral member of the RCC Leadership team,along with the Patient Care Manager, Supervisor, Clinical Educators, and Charge Nurses, you will have the opportunity to support staff toprovide the highest quality patient and family-centred care, while optimizing staff engagement and satisfaction.     The Clinical Leader is accountable for overseeing patient care and flow on the unit, including implementing processes that drive excellence in patient safety and quality outcomes. Position Summary:   The Nurse Practitioner (NP) is a Registered Nurse Extended Class with graduate education in Nursing, possessing advanced clinical and theoretical knowledge/skill with a focus on Gerontology. The NP is a professional who will function in an expanded role and will demonstrate a high level of autonomy as well as expert skill to formulate clinical decisions, manage acute/chronic illness, and promote wellness in Seniors.   In this position, the NP will provide care to patients in the Seniors’ Services program, which includes, but not limited to: out-patient services including outreach home visits and ambulatory clinic, supporting the emergency department, and geriatric in-patient services that includes the Seniors Care Unit. The NP will: conduct comprehensive geriatric assessments; diagnose; formulate management plan (including diagnostic, pharmacological and non-pharmacological therapies, education and counseling, clinical consultation); refer to/liaise with other community services; and, provide follow-up in. In addition, the NP may be involved in other domains of Advanced Practice Nursing that includes clinical, research, evaluation, leadership and collaborative activities (e.g. developing new standards of care, policies, quality initiatives, etc.).
