Job Description:
The In-Patient Medicine & ICU Program at THP consists of a total of more than 350 beds across the CVH & MH and is staffed by large interprofessional teams who support the care of complex medicine patients within the Mississauga-Halton LHIN. Our in-patient units offer first-rate care in an innovative environment supporting the physical, mental, emotional and spiritual needs of our patients. We are dedicated to building a diverse workforce that reflects the community in which we live and serve.
Reporting to a Clinical Manager, the ideal candidate will be focused on providing excellent patient centered care in a dynamic environment where he/she will be given the opportunity to deliver care with compassion, excellence and courage.
Required Qualifications:
- Graduate of a recognized college or school of nursing
- Current registration with the College of Nurses of Ontario and in good standing
- Able to function to the full scope of practice as per CNO
- Basic Life Support certification
- If working in an area with telemetry, successful candidates responsible for obtaining formal certification within six months of hire
- Demonstrated knowledge, experience and the ability to care for patients with the following care needs: initiation and maintenance of IV therapy, administration of medication above and below the drip chamber, blood and blood product administration, patient controlled analgesia, Enteral feeds, TPN, transfusion of blood products, central venous access devices CVADS (PICCs), blood glucose monitoring, naso-gastric tubes, wound management skills, routine infection control practices, normal and abnormal blood values, oxygen therapy, tracheostomies, and chest tubes.
- Computer skills including typing, sending and receiving email and working with Microsoft applications such as Word
- Satisfactory attendance and performance record required
Preferred Qualifications:
- Baccalaureate degree in Nursing preferred
- Minimum of two years acute care medical/surgical experience
- Completion of a physical assessment course
- Experience working with acute care medicine patient populations, including respirology, nephrology, cardiology patients requiring telemetry monitoring and geriatric patients
- Experience in providing and delivering patients and families with educational teaching
- Demonstrates effective interpersonal, communication, organizational and problem solving skills
- Demonstrates patient and family-centred care values
- Evidence of on-going professional development
- Experience and knowledge with Meditech and/or Sunrise Clinical Manager clinical application systems

Position Summary:
- Transports patients via bed, stretcher or wheelchair to and from various locations (i.e. patient unit to x-ray)
- Assists patients in and out of bed, stretcher or wheelchair
- Transports laboratory specimens from patient areas to specified laboratory
- Transports deceased patients by following protocols and delivering them to the mortuary

The Role
The Development Officer, Direct Marketing & Annual Giving is a full-time position within the Marketing and Community Engagement team at Trillium Health Partners Foundation. Reporting to the Manager, Direct Marketing & Annual Giving, the Development Officer, Direct Marketing & Annual Giving will work with the Manager to manage a portfolio of programs designed to support key business objectives including donor acquisition, donor retention, and revenue generation.
The successful candidate will have experience managing Annual Giving and Direct Marketing programs will and quickly build rapport with a number of vendors and internal relationships to achieve success. Preference will be given to candidates with prior experience in a hospital foundation, health-related charity or another health care or non-profit context.
PRIMARY RESPONSIBILITIES
- Responsible for contributing to sizeable annual fundraising targets across programs that include: Tribute Programs (In Honour, In Memory), Staff Giving, Direct Mail (including a Gift Catalog and Newsletters), Monthly Giving (including Door-to-Door Fundraising and Telemarketing) and mass digital fundraising (email, organic & paid social, search, display, etc.)
- Ensures programs are executed with a multi-channel approach and considers, email, social media & digital fundraising tactics, in collaboration with marketing & communications team.
- Continually measures and improves user experience for digital fundraising channels and ensures that all digital fundraising efforts support brand strategy.
- In collaboration with Director and Manager, supports the execution of comprehensive, measurable testing plans; a DM/email testing roadmap and the execution of A/B tests to optimize key metrics including open, clickthrough, and conversion rates
- Recommends and develops tactics for growth and stewardship and regularly monitors program performance, and adjusts or re-evaluates program tactics to optimize programs as appropriate.
- Manages contact with donors including responses to donor inquiries, monthly donor cancellation and/or complaints and escalates issues as appropriate. Includes RE data entry to capture donor activity for tracking and reporting purposes and ensures all donor interactions are accurately recorded and updated in Raiser’s Edge.
- Assists in managing vendors and suppliers (for example: Canada Post, Direct Mail and Telemarketing agencies, etc.), ensuring budgets and deadlines are met based on agreed upon goals & KPIs. Reports and manages issues that arise with vendor to resolution.
- Responsible for project management; delivering projects on scope and on time or flagging issues if necessary.
- Executes documented business rules around mid-level donor management and the movement / upgrade between annual programs and major gift programs.
- Works with Corporate Services department to ensure timely, accurate and proper gift processing, data pulls & list management, including monitoring & evaluation of data.
- Builds strong, collaborative relationships within the Foundation, with key departments within the hospital and with various vendors that will be managed by this role.
- Champions an environment that ensures effective sharing of information and collaboration with other Foundation programs and teams.
- Committed to learning and is informed on best practices, new tools and technology in digital fundraising and marketing.
- Stays engaged and current with hospital updates, health care topics and news of relevance to Direct Marketing.

- 3+ years of Direct Mail and Email development and telemarketing program execution and measurement

The Role
Reporting to the Manager of Philanthropy, the Coordinator will support the Philanthropy team, in all major and transformational giving activity including cultivation through to close & stewardship. In addition to providing support to the Manager of Philanthropy
Primary Responsibilities
Support Philanthropy Team
- Perform various duties in support of major gift activity including: scheduling donor meetings/hospital tours, drafting correspondence, presentations/decks, speaking notes and supporting proposal development
- Completing prospect donor research, briefing notes & call reports
- Provide hands-on support during meetings and special events as required (i.e. meeting preparation, trouble-shooting, greeting donors)
- Design and coordinate graphic support material as needed including fundraising online support
- Enter and update actions, call reports & donor information in Raisers Edge
- Work closely with the marketing & communications team to coordinate the production and dissemination of digital assets as it related to the philanthropy team (for example, newsletters, landing pages, social media posts, etc.)
- Support MG cultivation events as needed
- Provide support for the Grateful Patient Program by acting as the lead for certain programmatic areas within the Hospital
- Support on the creation of presentation decks, material briefs and meetings pertaining to the Grateful Patient Program
- Provide support to the Senior Development Officer, Philanthropy, on the Planned Giving Program
- Provide ongoing support of philanthropy team members as required
- Provide support for cultivation events, webinars, cabinet members as needed
Administration of Employee 50/50 Program
- Lead the administration, execution, and coordination of the employee 50/50 program including weekly draws + Reporting + Coordination with Winner, Bi-weekly in house ticket printing, managing opt in and opt out from program, financial tracking and reporting, coordination with payroll and more.

Under the direction of the Employee & Labour Relations Manager, this role provides strategic human resources advice and guidance to the business through the translation of business strategies into human resources strategies that support the achievement of the Strategic Plan. The ICU at the Mississauga Hospital Site is approximately a 31 bed, level 3 medical/surgical critical care unit staffed by an interprofessional team which supports regional programs including the neuro patient population within the Mississauga-Halton LHIN. Job Description:
Under the leadership of the Manager, Discharge Planning, the Social Worker will work within a fast paced environment as a collaborative team member of an interdisciplinary team.
In addition to being one of the key points of contact for discharge planning to formulate, facilitate and coordinate discharge plans focused on the needs and goals of the patient and their family system they will also be responsible for:
- Employing critical thinking and creative problem solving to facilitate all discharge planning, including complex discharge planning situations;
- Providing in-depth discharged focused assessments, collaboratively creating the discharge plan with the team, the patient and their family and providing support, education and guidance through the implementation of the discharge plan;
- Facilitating a timely transition to the community; identifying safety gaps/challenges and addressing all barriers using most appropriate resources in partnership with internal and external partners;
- Assessing risk and need for psychosocial intervention;
- Providing psychosocial assessments to identify the need for and subsequent provision of clinical social work services and/or referral to Social Work.
The primary goal of Discharge Planning services is to support and enable patients and families to make timely transitions through the hospital system. The Social Worker does all of this in an effort with the goal of providing an exceptional patient and family experience. The Social Worker understands the Flow and Capacity challenges of a hospital system and works to achieve that the patients are in the right place, at the right time, receiving the right care.
Qualifications:
- Masters degree in Social Work required
- Current registration and membership with Regulated College;
- 2-5 years clinical experience in an acute care hospital setting;
- Experience in Discharge planning is essential;
- Demonstrated ability to conduct psychosocial assessments and counselling and/or demonstrated ability to assess risk and need for referral to Social Work for psychosocial assessment and intervention;
- Demonstrated excellence in interview skills and ability;
- Current knowledge of community resources and initiative to proactively search for new resources;

Dr. Laura Rosella, Stephen Family Chair in Population Health and Associate Professor at the University of Toronto is seeking a postdoctoral fellow. The successful applicant will be part of an applied research team collaborating to apply novel analytic methods with an integrated population health perspective. Specifically, the postdoctoral fellow will lead a project to understand how to use data to improve outcomes by conducting a comprehensive assessment of high needs patients in the Mississauga community. Primary responsibilities will include writing grants, developing the research proposal, creating research plans, conducting statistical analyses, creating data visualizations, maintaining study documentation, and assisting in preparing manuscripts and other knowledge translation outputs.
Key Responsibilities
- Carry out project-specific activities from project initiation to project completion (including designing studies, analysis, interpretation of results, drafting of deliverables, creating tables and figures)
- Contribute to the development of research study protocols, including study design and analytic procedures
- Apply statistical and machine learning methods to predict and plan population health outcomes that include premature mortality and health system use
- Critically appraise and synthesize literature, retrieve articles, and maintain citation databases
- Contribute to the preparation of abstracts, poster presentations, peer-reviewed publications and other relevant deliverables
- Assist in the preparation of submissions to Research Ethics Boards; monitor project timelines and maintain study documentation in accordance with institutional policies and procedures
- Co-supervise (with PI) research assistants, trainees and students, and assist in the training and orientation of new staff
- Attend and participate in project-related meetings, including research team meetings
- Assist with other research-related activities as needed
Qualifications
- PhD in epidemiology, public health, biostatistics, engineering, computer science or a related field within the last 2
- Demonstrated knowledge and experience with quantitative statistical analysis, epidemiological methods and data management, preferably using SAS, R, or python
- Experience in managing epidemiological and/or health services research projects; familiarity with the use of large health databases for the conduct of observational studies
- Experience with routinely collected health care data not necessary but considered an asset
- Strong organizational, time management and priority setting skills
- Strong oral, written, and interpersonal communication skills, with a demonstrated aptitude for report and presentation preparation
- Ability to function independently yet collaboratively within a team

The In-Patient Medicine & ICU Program at THP consists of a total of more than 350 beds across the CVH & MH and is staffed by large interprofessional teams who support the care of complex medicine patients within the Mississauga-Halton LHIN. Our in-patient units offer first-rate care in an innovative environment supporting the physical, mental, emotional and spiritual needs of our patients. Required Qualifications:
- Graduate of a recognized college or school of nursing
- Current registration with the College of Nurses of Ontario and in good standing
- Able to function to the full scope of practice as per CNO
- Basic Life Support certification
- If working in an area with telemetry, successful candidates responsible for obtaining formal certification within six months of hire
- Demonstrated knowledge, experience and the ability to care for patients with the following care needs: initiation and maintenance of IV therapy, administration of medication above and below the drip chamber, blood and blood product administration, patient controlled analgesia, Enteral feeds, TPN, transfusion of blood products, central venous access devices CVADS (PICCs), blood glucose monitoring, naso-gastric tubes, wound management skills, routine infection control practices, normal and abnormal blood values, oxygen therapy, tracheostomies, and chest tubes.
