- Preparation of cardiopulmonary bypass circuit for extra-corporeal support

Position Summary:
Reporting to the Clinical Manager of Pharmacy Services you will be an integral member of the Pharmacy team and be accountable for the pharmacotherapy for patients to ensure optimal patient care is achieved. Position Summary:
As a member of the care giving team on the Patient Services units, assists the Nursing staff with the provision of patient care. Position Summary: Sexual Assault and Domestic Vioelnce Srevices Mandate:
Trillium Health Partners operates the regional program for the medical/forensic care of:
- women and men 12 years of age or older who have been recently sexually assaulted
- women and men 16 years of age or older who have incurred recent physical injuries as a result of domestic violence
- children under the age of 12 who have reported sexual assault/abuse
Clients may access this 24-hour service through the Mississauga Hospital Emergency Department and are cared for by a specially trained nurse. A process of: assessment, treatment, documentation (may include photography), safety planning and referral is undertaken by the nurse in the safety and security of a private unit separate from the Emergency Department called Chantel’s Place.
The Principle Responsibilities include:
Clinical Practice
- Demonstrates advanced knowledge and analysis of nursing practice issues, within the scope of NP practice

- Performs a comprehensive history that records and interprets the pertinent medical, family and psychosocial history

Position Summary:
Reporting to the Clinical Manager – Peri-operative Care, the PACU RN collaborates with the inter-professional team to provide airway assessment and management, cardiac and hemodynamic monitoring, pulse oximetry, and ongoing nursing assessment, diagnosis and intervention to all patients in the PACU.
Qualifications:
- Graduate of a recognized College or School of Nursing
- Current certification of competence from the College of Nurses of Ontario is required
- Baccalaureate degree preferred
- Current B.C.L.S., Coronary Care 1 and Coronary Care 2 OR Critical Care Certificate required
- Minimum 1-2 years of recent experience in PACU or Critical Care required

Position
The Carlo Fidani Regional Cancer Centre at the Trillium Health Partners Credit Valley Hospital is seeking a permanent, full-time Medical Physicist, who will work in collaboration with the medical physics team to provide clinical support. The Credit Valley Hospital is a teaching hospital of University of Toronto, and the radiation oncology program is recognized by its high quality, high efficiency and innovations in safety and patient-centered care. You are a medical physicist who demonstrates passion and innovation for advancing medical physics.
Opportunity
The Radiation Treatment Program (RTP) of the Carlo Fidani Regional Cancer Centre is a fully integrated and electronic program that treats >4000 patients per year. The Cancer Centre is equipped with six Varian TrueBeam linear accelerators and two CT simulators (Canon Aquilion Exceed Large Bore and Philips Big Bore Brilliance). A fully networked infrastructure for treatment planning on Varian Eclipse on a Citrix platform for seamless treatment planning and delivery. The Varian ARIA oncology information system provides full electronic data for Radiation Oncology. Current clinical programs include Linac-based Stereotactic Radiosurgery utilising HyperArc as well as a state-of-the-art Brachytherapy suite (Elekta Nucletron, Flexitron afterloader). Plans are underway for an expansion of the Cancer Centre, infrastructure includes the addition of two Varian Halcyons with HyperSight, one Varian Ethos 2.0 and an Xstrahl 200 Orthovoltage X-ray therapy machine.
The Medical Physics department is integrated in the RTP in relation to its clinical, education and research mandate as defined by the mission and objectives of Ontario Health-Cancer Care Ontario and Trillium Health Partners. The department currently includes twelve medical physicists, four full-time and two part-time physics associates, three electronics technologists, as well as a shared administrative assistant; all of whom report to the Head of Medical Radiation Physics. An active Medical Physics Residency Program (one position) is supported through affiliation with the University of Toronto Department of Radiation Oncology (UTDRO) CAMPEP accredited residency program.
The successful candidate will posses the ability to work well in a strong, team-oriented environment and be responsible for all clinical and technical aspects of the medical physics program. Clinical activities will include acceptance and commissioning of new equipment, chart checking, quality assurance, treatment planning and delivery support, teaching, as well, research and development activities of the medical physics department. Academic appointment may be sought through the UTDRO.
Qualifications
- MSc or PhD in Medical Physics
- Successful completion of a CAMPEP accredited residency training program in Radiation Oncology Physics
- Membership in the Canadian College of Physicists in Medicine (CCPM) or equivalent is preferred and or required within two years of entering this position
- Strong organizational, interpersonal and communication skills

A 37 bed neuro medical surgical ICU that is also the regional stroke and neurosciences center. Specifically as a stroke and vascular center—we see patients requiring hyperacute stroke care, post endovascular thrombectomies, abdominal aortic aneurysms. Insight Health Solutions (IHS) is a provider of third-party health services to a variety of sectors. Embedded within the entrepreneurial program THP Solutions, IHS works closely with our internal stakeholders to deliver high quality, expedited assessment, treatment and diagnostic services.
Job Description:
The Rapid Access Clinics (RAC) – Hip/Knee and Low Back Pain are programs funded by the Province of Ontario’s Ministry of Health (MOH). The RACs are innovative models of care in which patients receive rapid assessment, education, and evidence-based recommendations in the management of their musculoskeletal conditions.
As a clinician in the RACs, the Advanced Practice Provider delivers expert-level clinical practice in an outpatient setting within an interprofessional model of care. In collaboration with orthopaedic surgeons and other members of the team, the APP facilitates timely access to quality musculoskeletal (MSK) care and provides referrals to appropriate treatment and specialist consultations for patients referred to Trillium Health Partners. The APP is a champion for best practice, and innovative care models and contributes to quality improvement initiatives to achieve safe, effective, efficient care. In addition, the APP acts as the first and primary point of contact for consultation and assessment of referred patients; provides a defined program of care to the patient with an emphasis on education, self-management and health promotion strategies; develops a collaborative care plan with the patient and their primary care provider and documents progress against the care plan; and identifies signs and symptoms requiring a change in the plan of care, diagnostic imaging/testing, or referral to a physician specialist.
Please note successful candidates must complete both practical and theoretical training (online/in-person) in LBP topics at the region’s hospital hub.
Educational Requirements:
- At least 4 years of clinical experience following completion of an accredited, entry-to-practice program in Physical Therapy, or Chiropractic Medicine, required.
- Completion of the Advanced Clinician Practitioner in Arthritis Care (ACPAC) program required.
- Minimum 5 years of recent clinical expertise, working in conjunction with Orthopaedic Surgeons in the care of pre- and post-operative orthopaedic patients and in the care of arthritis/MSK diagnoses required.
- Minimum 1 year of clinical experience in the musculoskeletal-spine area,
- Member in good standing of respective professional/legislative college at the provincial level, required.
- Member of related national/provincial professional association (e.g. OCA, OPA), preferred.
Key Responsibilities
- Acts as the first and primary point of contact for patients referred to the RAC for consultation and assessment.
- Provides specialized care, education, and counselling for RAC patients.
- Performs a medical history, physical and health status for patients.
- Conducts comprehensive assessment and communication of a diagnosis.
- Determines the suitability and eligibility of patients for certain types of surgery.
- Differentiates between inflammatory and non-inflammatory conditions.
- Has a broad understanding of indications for surgery and risks/benefits for surgery.
- Identifies signs and symptoms of undiagnosed medical conditions requiring a referral to diagnostic imaging/testing and/or referral to a physician specialist.
- Establishes, communicates, and documents clinical impressions/medical diagnoses within defined scope of practice.
- Initiates communications with and directs appropriate referrals to orthopaedic surgeons based on assessment finding and referral guidelines
- Reviews appropriate imaging to support assessment, diagnosis and decision-making
- Consults with orthopaedic surgeons and others on the care team, to establish appropriate care for patients, including those with complex presentations and needs
- Serves as a clinical consultant for the patient population
- Provides education and recommendations to patients, families and their primary care providers to support the management of their condition and optimize function in keeping with best practice guidelines
- Liaises with referral sources and community service providers as required
- Contributes to the development, evaluation and improvement of team-based care and practice innovations
- Acts as an ambassador for the program Supervision of students as required

Job Description:
The Inpatient Medicine & ICU Program (the Program) consists of a more than 350 budgeted beds across the CVH & MH and is staffed by inter-professional teams who support the care of critically ill and complex medicine patients within the Mississauga-Halton LHIN. Our teams provide high quality-patient centered care in an innovative environment to support the physical, mental, emotional and spiritual needs of our patients. We are dedicated to building a diverse workforce that reflects the community in which we live and serve.
Reporting to a Clinical Manager within the Program, the ideal candidate will be focused on providing patient centered care and will be given the opportunity to deliver care with compassion, excellence and courage.
Required Qualifications:
- Bachelor or Master’s Degree in Physiotherapy
- Certificate for Independent Practice with the College of Physiotherapists of Ontario
- Rostered with the College of Physiotherapists of Ontario for tracheal suctioning and administering a substance by inhalation (or evidence of becoming rostered within six months of hire)
- Experience and ability to work in an inter-professional team environment
- Experience in working effectively and collaboratively with Occupational Therapy Assistants/Physiotherapy Assistants
- Basic Cardiac Life Support certification (or evidence of completion within three months of hire)
- Excellent attendance record
- Exceptional interpersonal and communication skills (verbal and written), including problem solving, negotiation and ability to have difficult conversations
- Effective Team player
Preferred Qualifications:
- Experience working in in-patient medicine and/or intensive care units
- Experience in providing education to patients and families in individual and group settings
- Evidence of on-going professional development
- A second language is considered an asset in serving our community
Job Responsibilities:
- Providing physiotherapy coverage for the Program across sites as assigned (e.g. coverage on weekends, statutory holiday coverage, during planned and unplanned absences)
- Planning and directing physiotherapy services, including assessment, treatment and evaluation
- Facilitating appropriate care planning with the inter-professional team to support seamless transitions for our patients
- Practicing in compliance with legislation, hospital policies and procedures, professional regulatory standards and physiotherapy best practices
- Supporting and participating in the development of program and unit-level objectives, policies, procedures and quality assurance programs
- Completing workload measurement reporting within a timely manner
- Provide clinical teaching, mentoring and/or supervision for PT students. Engage with inter-professional education experiences with students of all health disciplines
