Position Summary:
- Order and stock housekeeping supplies
- Distribute and store supplies (e.g. Linen, housekeeping supplies, food) in the appropriate room or area
- Maintain inventory of equipment in appropriate area on unit (e.g. Wheelchairs, stretchers)
- Transport patients to and from other departments in the hospital (e.g. X-Ray)
- Transport patients and/or furniture within the unit
- Deliver and set up patient meal trays
- Collect soiled trays and place them in meal cart
- Distribute patient nourishments on the unit
- Clean patient rooms, lounges, offices and common areas on a daily basis
- Clean utility rooms, pantries, corridors and nurses’ station
- Empty garbage, sanitize garbage containers, clean up spills and maintain floors (e.g. Dry mop, wet mop, buffing)
- Strip and make unoccupied beds and stretchers
- Uses chemicals and cleaning agents
- Maintain open and timely communication with team members and management staff, including using a pager as require
- Performs other related duties as required
Key Qualifications:
- Minimum Grade 12 education and/or equivalent in support services role preferred
- Ability to communicate with patients and co-workers
- Ability to read and write and follow instructions
- Must have the physical requirements to complete the job duties
- Must be able to work all shifts (Days, Evenings, Nights and Weekends)

Position Summary:
Reporting to a Clinical Manager, the ideal candidate will be focused on providing excellent patient centered care in a dynamic environment where he/she will be given the opportunity to deliver care with compassion, excellence and courage.
Job Description:
- Provision of nursing care for patients within an interprofessional environment, including assessment, treatment, teaching and education
- Teaching and coaching to patients and families
- Commitment to a healthy, safe and respectful workplace
- Practicing in compliance with provincial legislation, hospital policies and procedures; professional regulatory standards; and best practices within the nursing profession

Required Qalifications:

This position will work on the Night Operations team (NightOps) working after hours, weekends and statutory holidays exclusively. Shifts are typically 1600-0000hrs and 0000-0800hrs throughout the week days combined with 12-hour shifts on the weekend such as 0800-2000hrs and 2000-0800hrs. The successful candidate will be expected to work a combination of week day and week end shifts in a typical week.
JOB DUTIES AND RESPONSIBILITIES
Working collaboratively with staff, your job duties and responsibilities will include but are not limited to:
- Provide 2nd level technical support to users throughout the organization; Assists users in solving problems using available hardware and software tools;
- Document issues and troubleshooting steps in the ServiceNow Incident Management system and knowledge base;
- Deploy hardware including printers, mobile devices, laptops/desktops etc;
- Support mobile devices such as iPhones and iPads using Intune mobile device management (MDM) software
- Provide assistance for troubleshooting of hardware, software and network issues;
- Performs proactive and preventive maintenance on end user devices;
- Common administration activities with Active Directory to support users as required;
- Provide end-user training as appropriate;
- Provide support at all THP sites. Travel between sites may be required.
- Actively seek opportunities for service improvement, recognizing the needs and expectations of our customers;
- Provide knowledge transfer and mentoring of other team members when required;
- Other duties as assigned.
WORKING ENVIRONMENT
Working within a multi-site organization; this role is part of a team providing service to the organization on a 24x7x365 schedule. The successful candidates may be required to work shifts outside of regular business hours on a rotational basis. Work locations can vary between all Trillium Health Partners locations including remote physician office locations.
JOB QUALIFICATIONS
Knowledge
- University Degree or College Diploma in a computer related field or equivalent work experience
- Windows 10 required
- InTune MDM required
- Apple & Android Mobile Phones required
- ServiceNow ITSM experience required
- ITIL Foundations certification preferred
- CompTIA A+ certification preferred
- Microsoft MCSA, 365, or similar Microsoft certification
Experience
- Experience in a health related field required
- 5+ years’ experience in computer deskside support
- 5+ years Active Directory administration
- 2+ years using software deployment and inventory tools such as Microsoft SCCM and InTune
- Experience supporting Windows 10 & Microsoft Office 365 is required, certification is preferred

- Strong client computing experience e.g. desktops, laptops, printers, mobile devices (iPhone & Android), multifunction devices, desktop phones, voicemail;
- Solid skills in supporting common end user applications including Microsoft Office, Internet Explorer, Outlook etc.;
- Strong troubleshooting skills with focus on root cause analysis;
- Experience with tools and technologies such as Microsoft SCCM, Bomgar, Active Directory, Microsoft Exchange;
- Knowledge of networking technologies such as VPN, TCP/IP, and wireless;
- Experience with IT Service Management tools such as ServiceNow
- Strong verbal and written communications; must be able to communicate with individuals with all levels of technical and non-technical skills

Working collaboratively with staff, your job duties and responsibilities will include but are not limited to:
- Provide 2nd level technical support to users throughout the organization; Assists users in solving problems using available hardware and software tools;
- Document issues and troubleshooting steps in the ServiceNow Incident Management system and knowledge base;
- Deploy hardware including printers, mobile devices, laptops/desktops etc;
- Support mobile devices such as iPhones and iPads using Intune mobile device management (MDM) software
- Provide assistance for troubleshooting of hardware, software and network issues;
- Performs proactive and preventive maintenance on end user devices;
- Common administration activities with Active Directory to support users as required;
- Provide end-user training as appropriate;
- Provide support at all THP sites. Travel between sites may be required.
- Actively seek opportunities for service improvement, recognizing the needs and expectations of our customers;
- Provide knowledge transfer and mentoring of other team members when required;

WORKING ENVIRONMENT
Working within a multi-site organization; this role is part of a team providing service to the organization on a 24x7x365 schedule. The successful candidates may be required to work shifts outside of regular business hours on a rotational basis. Work locations can vary between all Trillium Health Partners locations including remote physician office locations.

Knowledge

Trillium Health Partners (THP) is seeking a Project Manager to join the Health Hubs team.
The Health Hubs team supports the hospital’s strategic plan by leading the development of Health Hubs, or campuses of care, that purposefully co-locate, connect and integrate healthcare services. By connecting the hospital with long-term care and community services, this creates the opportunity to focus on connected care models that are designed to target the right services to the population needs. This then build system capacity, improves system flow, alleviates hospital pressures and supports better access to care overall.
Project Managers within the Health Hubs team will work collaboratively with teams across the organization to support transformative initiatives, processes and policy development designed to achieve the goals identified in the hospital’s strategic plan including Trillium HealthWorks and Health Hubs projects. Here’s what you will get to do:
- Lead the evaluation and execution of corporate development initiatives including mergers, acquisitions, divestitures and other strategic initiatives and partnerships to advance innovate models of integrated and seamless care across the health system;
- Lead business case development, financial analysis, due diligence, transaction negotiations and partnership management to advance campuses of care;
- Provide project management leadership to a multi-disciplinary team to ensure successful delivery of all phases of the project life cycle to support the creation of campuses of care;
- Monitor and report on progress, problems and solutions of the project to all stakeholders and escalate issues as appropriate;
- Provide strategic oversight and guidance to support project alignment, corporate decision-making within THP’s governance structure and complex deliverables;
- Lead the development of materials, such as reports, briefing notes, requests for proposals, presentations, data analyses and communications for the project(s) for all levels of the organization’s partners, government and external partners, and the Board of Directors;
- Lead the development and dissemination of project control documents, including status reports, risk / issue analyses and reports and project plans;
- Lead and / or support working groups and committees that are integral to the success of the project(s);
- Engage and communicate with stakeholders on an ongoing basis and ensure changing resource and project requirements are met and understood;
- Liaise with internal and external stakeholders including regional, provincial and/or private sector partners to ensure the successful completion of project deliverables.
- Support project research, literature reviews and data / information gathering to enhance subject-matter knowledge relevant to the project; and
- Lead or support other projects, as required, within the Strategic Projects Portfolio, which may include initiatives related to advancing THP’s Strategic Plan, Major Projects, virtual care as well as projects THP may engage on with external partners.
Qualifications:
- Ideal candidates must have an undergraduate degree; business, life sciences, or related discipline preferred.
- Masters degree in public health/healthcare administration, business administration, or equivalent experience considered an asset.
- Minimum 3-5 years of experience in project management in healthcare environments such as hospitals, long-term care, government or private sector required
- Strong understanding of the hospital’s strategic plan
- Strong analytical, problem solving and critical-thinking abilities
- Adaptability, interpersonal and collaboration skills are essential for this role

The Inpatient Medicine & ICU Program at THP consists of a total of more than 350 beds across the CVH & MH and is staffed by large interprofessional teams who support the care of complex medicine patients within the Mississauga-Halton LHIN. Required Qualifications:
- Graduate of a recognized college or school of nursing or equivalent (e.g. certificate RPN with added courses in medication administration, phlebotomy, and health assessment)

Qualifications:
- Current certification or eligibility for certification of competence from the College of Nurses of Ontario is required
- Candidate will be a diploma graduate of a recognized college or school of nursing or equivalent (i.e., certificate RPN with added courses in medication administration, phlebotomy, and health assessment)
- Oncology Nursing – Registered Practical Nurse (Post-Graduate) preferred
- A physical assessment course required

Department/Program/Unit Summary:
The In-Patient Medicine Program at THP consists of a total of more than 350 beds across the CVH & MH and is staffed by large interprofessional teams who support the care of complex medicine patients within the Mississauga-Halton LHIN. Our in-patient units offer first-rate care in an innovative environment supporting the physical, mental, emotional and spiritual needs of our patients. Position Summary:
Reporting to a Clinical Manager, the ideal candidate will be focused on providing excellent patient centered care in a dynamic environment where he/she will be given the opportunity to deliver care with compassion, excellence and courage.
Job Description:
- Provision of nursing care for patients within an interprofessional environment, including assessment, treatment, teaching and education
- Teaching and coaching to patients and families
- Commitment to a healthy, safe and respectful workplace
- Practicing in compliance with provincial legislation, hospital policies and procedures; professional regulatory standards; and best practices within the nursing profession
- Maintaining professional competence by completing regular professional development (including annual training/education as indicated)
Required Qalifications:
- Graduate of a recognized college or school of nursing
- Registered in good standing with the College of Nurses of Ontario (CNO) or currently meet the eligibility criteria to register in good standing in the Temporary Class or General Class with the CNO
- Able to function to the full scope of practice as per CNO
- Basic Life Support certification
- If working in an area with telemetry, successful candidates responsible for obtaining formal certification within six months of hire
- Demonstrated knowledge, experience and the ability to care for patients with the following care needs: initiation and maintenance of IV therapy, administration of medication above and below the drip chamber, blood and blood product administration, patient controlled analgesia, Enteral feeds, TPN, transfusion of blood products, central venous access devices CVADS (PICCs), blood glucose monitoring, naso-gastric tubes, wound management skills, routine infection control practices, normal and abnormal blood values, oxygen therapy, tracheostomies, and chest tubes.
- Computer skills including typing, sending and receiving email and working with Microsoft applications such as Word
- Satisfactory attendance and performance record required
Preferred Qualifications:
- Baccalaureate degree in Nursing preferred
- Minimum of two years acute care medical/surgical experience
- Completion of a physical assessment course
- Experience working with acute care medicine patient populations, including respirology, nephrology, cardiology patients requiring telemetry monitoring and geriatric patients
- Experience in providing and delivering patients and families with educational teaching
