Locates staff, pages healthcare professionals as required. - Open, sort and distribute incoming mail and other materials.   New referrals processing and information management - Receives and logs incoming referrals, reviews for completion, follows up as required. - Creates and maintains charts. Ensures required forms are received, completed correctly and put into patient file. - Enters patient information and appointments into required databases, updates with appointment and other medical information - Makes reminder phone calls for next day appointments - Assists new patients completing required questionnaire, responding to questions within scope of authority. - Prepares and updates patient charts with lab reports, test results and other pertinent information. Scans and copies chart and other information as required. - Ensures patient discharged are processed and charts are sent to Health Records   Patient Administrative Services - Books outpatient new and follow up appointments, triaging referrals as appropriate. Cancels appointments and follows up with other patients for rescheduling of visit. - Explains test/visit preparations as required within scope of authority. - Liaises with internal departments and external healthcare facilities to book/cancel tests, appointments. - Books interpreters for patients as needed. - Arranges for transportation for patients as required (eg. those staying in nursing homes). - Transcribes and enters physician orders including medication, lab and imaging orders and other patient information into appropriate system - Enters data including appointment requests, discharge documentation. nursing acuity/workload assessments as required. - Notifies appropriate personnel of patient movement, cancellations, appointments and other patient information in a timely and accurate manner.   Ensures efficient clerical administration and operation of the department within scope of authority - Prepares clinic lists, identifies orders needed as required. - Enters and maintains payroll records (include sick and vacation requests). - Creating and managing Physician booking, Allied Health grids (calendar management) as required. - Transcribes diagnostic reports, assessments, minutes and correspondence - Submits billings to CCAC, WSIB and other organizations as required - Responds to and prepares written correspondence in reply to requests for patient information from external institutions within scope of authority - Request/receives reports from external healthcare facilities as directed - Prepare CD's and other patient information for review by the healthcare professional - Requisitions and maintains office/medical supplies; counts and labels inventory as required. May be required to stock unit carts and/or supply areas. - Processes routine financial transactions including unit supply/inventory purchase and charges for services/items not covered by patient's healthcare plan and provides invoice. Liaises with Accounts Receivable as required.   Performs cross-functional, and/or, other duties consistent with the job classification, as required. Works in compliance of the Occupational Health & Safety Act and its regulations, reporting hazards, deficiencies and contravention’s of the Act, in a timely manner. The project is underway and in its final phase before implementation with go-live in the near future. After the completion of the OneTHP project, a team will continue to oversee the maintenance, support and continued evolution of the Epic platform working closely with Information Services (IS) team and leadership.   The Position   The Director of Technology and Integration role will have two distinct phases with different accountabilities as part of each phase. This position will report to the AVP, OneTHP Executive Lead in both phases, the project phase and operations phase to support the Epic Hospital Information System as installed at THP.   The individual who fulfills this position is an experienced leader combining excellent leadership skills with managing teams, extensive knowledge of information technology and integration, and the ability to balance operational needs and stability, with strategic thinking. This individual will need to have strong collaboration skills to work closely with other leaders within the OneTHP structure as well as other departments including Information Services (IS).   During the project phase, the Director will work collaboratively with other OneTHP project leaders, IS Leaders and Epic leaders to ensure project milestones are met, planning activities have been completed and activities executed per approved plans. Summary:   Reporting to the Program Director of Oncology at Trillium Health Partners (THP), the Nurse Practitioner will continue to develop and provide clinical support to the Complex Malignant Hematology (CMH) program to decrease patient wait times, improve access to care, decrease fragmentation of care and coordinate services, thereby enhancing a positive patient experience closer to home. As a nursing leader, this new role will be involved in program development, program planning, education, Clinical service delivery as well as research, and evaluation.   Responsibilities:   Program Development and Evaluation: - In partnership with Physician and administrative leadership, continue to develop and implement the CMH program for THP including the care delivery model, policy and practice standards, protocols and guidelines - Develop appropriate evaluation of the CMH program including CCO performance metrics to monitor quality and best practice - Utilize evidence-informed guidelines to guide the development of quality of care improvement strategies in the trajectory of patient care. - Inform the development of key clinical pathways to improve the quality of care, reduce risks, increase patient satisfaction and increase the efficiency in the use of resources - Design key CMH program elements so that it is positioned strategically within the oncology program at THP, regionally and provincially - Further develop key partnerships with other regional cancer programs and CCO. - Conduct a readiness assessment for the provision of outpatient consolidation chemotherapy. - Provide leadership and assist with professional development of team members through mentoring and participating in formal and informal education initiatives. - Identify quality initiatives that will strengthen and further develop nursing professional practice. We are one of 14 Regional Cancer Programs created by Cancer Care Ontario to ensure cancer care is delivered according to province-wide quality and performance standards. Aligned with Ontario Health (CCO) Ontario Cancer Plan V, the MHCW Regional Cancer Program is dedicated to delivering the highest quality, compassionate, person-centred cancer care and treatment that is designed to improve patient outcomes and reduce the burden of cancer.   Overview of the opportunity:                                                      Reporting to the Regional Director of the Regional Cancer Program, the Regional Program Coordinator will work alongside the leadership team to manage and coordinate functions, including:   Managing Information Flow & Ensure Strategic Decision Making - Liaising with RCP leadership to provide direction and guidance to partnering hospitals in alignment with Ontario Health (CCO), Ontario Cancer Plan V, and performance indicators. Managing RCP Program Structures & Meetings - Collaborating with key hospital partners to develop strategic agendas for leadership committee meetings, which includes developing work plans to ensure effective decision making and agenda management. - Liaising with Program Leaders and Administrative Assistants to support accountability and meeting logistics. Preparing Communications, Briefings, Presentations & Reports - Performing external and internal analysis across the region, leveraging business acumen and writing, to create reports, presentations, speeches, letters and other documents for both internal and external use. - Leading the development of regular reports on behalf of the Regional Cancer Program (RCP), including quarterly reports to ensure partners are informed. - Developing and managing communications by responding to requests, gathering additional information, triaging and referring to program leadership as appropriate to ensure requests are addressed in a strategic, timely and coordinated manner.
