The Unit Coordination Assistant provides clerical support for reception; new referrals processing and information management; patient adminstrative services, ensures the clerical administration and operation of the patient services department/clinic; and performs other duties consistent with the job classification as required.   JOB DUTIES & RESPONSIBILITIES:   Reception - Greets patients, visitors and staff, directing them to appropriate personnel and/or destination/room, providing assistance as required.   - Completes positive patient identification on arrival in unit. Merges forms from the patient with printing from registration and Pre-Surgery Clinic to complete health record.. - Explains hospital/department policy and procedures to patients, visitors and staff as necessary (including infection prevention and control). - Answers the telephone, sends/received faxes, responding to enquiries within scope of authority, or, directing to most appropriate person. - Takes, prioritizes and relays messages for staff and/or patients. Locates staff, pages healthcare professionals as required. - Open, sort and distribute incoming mail and other materials. New referrals processing and information management - Creates and maintains charts. Ensures required forms are received, completed correctly and put into patient file. - Enters patient information and appointments into required databases, updates with appointment and other medical information - Assists new patients completing required questionnaire, responding to questions within scope of authority. - Prepares and updates patient charts with lab reports, test results and other pertinent information. Scans and copies chart and other information as required. - Ensures patient discharged are processed and charts are sent to Health Records Patient Administrative Services - Books outpatient new and follow up appointments, triaging referrals as appropriate. Cancels appointments and follows up with other patients for rescheduling of visit. - Explains test/visit preparations as required within scope of authority. - Liaises with internal departments and external healthcare facilities to book/cancel tests, appointments. - Books interpreters for patients as needed. - Arranges for transportation for patients as required (e.g. those staying in nursing homes). - Transcribes and enters physician orders including medication, lab and imaging orders and other patient information into appropriate system - Enters data including appointment requests and discharge documentation. - Notifies appropriate personnel of patient movement, cancellations, appointments and other patient information in a timely and accurate manner. The MHCW Regional Cancer Program is one of 14 Regional Cancer Programs responsible for championing and influencing system transformation to drive performance of the cancer system and the region.  The mission of the Regional Cancer Program from the strategic plan is:  “Exceptional, compassionate care for people impacted by cancer”. “Together, we will:  Support the whole person; Connect and coordinate care; Optimize quality of care and experience; and, Excel in performance and sustainability.”   The Position   As the Regional Program Director, Mississauga Halton Regional Cancer Program, you will report to the Regional Vice President, Ontario Health (CCO) and Senior Vice President, Patient Care Services, THP. A broad and influential leadership role that includes both a regional and programmatic mandate, you will ensure lead regional planning and performance improvement in one of the largest regional cancer programs in the province.   The Regional Director leads an inter-professional team providing system and regional leadership, relationship management, performance management and financial and risk management for cancer services across the region.  Cancer Services include:   Systemic Treatment, Radiation Therapy, Surgical Oncology, Cancer Imaging, Pathology & Lab Medicine, Screening, Prevention, Psycho-social Oncology, Patient Education, and Survivorship.  Additionally the role provides integration and engagement/support for the Regional Palliative Care Network, Regional Endoscopy Services,  Complex Malignant Haematology, Primary care, Integrated Cancer Screening (including OBSP sites), Regional Cancer Informatics and Patient Experience across 2 former LHINS, 4 hospitals (10 hospital sites), and 34 OSBP sites. You will ensure the delivery of strategic initiatives are aligned with quality care for patients and their families, and will play a vital role in building and sustaining excellence in regional service delivery.   Key Accountabilities for the Regional Director are: - Develop and implement key strategic initiatives aligned with the Ontario Cancer Plan linked with the MHCW RCP’s strategic plan, goals and objective - Build high performing teams that implement local and regional aspects of the Ontario Cancer Plan - Implement the clinical quality improvement agenda of the provincial program for all cancer clinical service delivery area - Establish and maintain operational partnerships in the region necessary to advance cancer system performance and priorities as well as jointly develop solutions to cancer system and related challenges - Together with the RVP, facilitate cancer service capacity planning within the Regional Cancer Program and as part of province-wide cancer planning. - Together with the RVP, build and maintain relationships with a range of care networks, community providers and cross-sector agencies to advance a partnership strategy that is integrated and outcome-focused - Support hospital operations partners around contract management including adherence to data and volume requirements, and quality and performance standards. - Support performance oversight of the cancer system in the region (including performance of volumes, data, access safety and quality) and lead efforts to improve regional performance - Champion provincial cancer standards to ensure these are put into practice at the point of care, through dissemination of standards and compliance monitoring. - Support the RVP to oversee cancer related funding and health human resources (HHR) within the RCP including development of physician HHR plans and recruitment strategies, supporting partner hospitals with QBP funding/volumes optimization to serve our population. - Facilitate patient and family involvement in committees for planning, and evaluating activities and services. - Collaborate and liaise with Ontario Health (CCO) and the Regional Directors throughout the province through participation and leadership on provincial committees and projects; and advance the overall care experience for persons with cancer in our community. Key Experience/Qualifications:   The successful candidate will possess the following experience, skills, attributes, and capabilities: - 7+ years of progressive clinical operations leadership experience in an acute-care hospital or related healthcare environment - Experience in overseeing large scale clinical programs, in Oncology and/or with regional service delivery models would be considered a strong asset - Knowledge of strategic and operational planning, management principles and business case development - Knowledge of quality improvement, financial practices and risk management principles - Demonstrated strong financial and analytical skills - Demonstrated leadership, communication and interpersonal skills that can be employed in a complex environment with multiple stakeholders - History of building constructive and effective relationships and gaining credibility in other organizations and within those organizations across disciplines, divisions, departments and programs. - Demonstrated ability to think strategically and implement sophisticated programs and initiatives - Excellent oral and written communication skills - Highly motivated and enthusiastic team player who thrives in ambiguity - Strong influencer.
