Unit Summary:
THP is a Regional Cardiac Program providing services and care in collaboration with our Regional Partners. The CCU at Mississauga Hospital is a 14 bed Level 3 Critical Care Unit working closely with our Cath Lab and Cardiology department to care for a wide variety of complex cardiac patients. Being a Regional Program, our unit is fast paced, high energy and highly skilled, the perfect place to advance your cardiac knowledge and skill while being part of a highly effective team.
Qualifications:
THP is committed to internal talent, and therefore priority will be given to internal applicants.
- Registered in good standing with the College of Nurses of Ontario (CNO) or currently meet the eligibility criteria to register in good standing in the Temporary Class or General Class with the CNO
- Currently enrolled and have already commenced a recognized Critical Care Certificate Program
- BCLS required
- ACLS required within 6 months of hire
- Current Coronary Care Level 1 and 2 Certificate Required
- Recent experience and exposure in Intensive Care nursing preferred
- Advanced physical assessment skills

Job Description:
Reporting to the Manager, Workforce Management Office (WMO), the successful candidate(s) will lead a team of Scheduling Clerks accountable for staff scheduling, shift and absence management and timekeeping for nursing, allied health and clerical staff across clinical programs at all sites of Trillium Health Partners. The WMO will support a team focused approach in which the Scheduling Clerks will work with their peers and with Patient Care Managers/designate(s) to collaboratively ensure 24/7 adequate staffing coverage across clinical programs (all sites) to optimize the quality of patient care delivered. This is an opportunity to be a part of a new and exciting initiative for THP that is focused on creating OneTHP – One People Experience. The WMO will operate 24/7 to provide ongoing operational coverage for staffing relief.
The WMO is a new department within THP that will be established between Spring 2024 and Fall 2024 (the Project Phase) in a phased approach. During this period, the WMO Team Leaders will be accountable to the Manager and assist with recruitment, onboarding and training of new staff, facilitation of team building, and skill acquisition set up and maintain ongoing day to day operations of the WMO. Accountability will include the transition of clinical program scheduling development, maintenance and absence management through the WMO prior to project go-live. Upon go-live with the new scheduling and shift management system (UKG Dimensions and AndGo Systems) in Fall 2024, the Team Leaders will transition from project phase to operations along with the day to day office responsibilities.
The successful candidate(s) will excel in operational effectiveness; are committed to patient-centred design; and have a passion for leading workplace environment transformation within an interprofessional acute care setting. The Team Leaders will work in partnership with Patient Care Managers (PCMs) and delegates to understand the staffing and daily operational needs and provide oversight and guidance to WMO scheduling coordinators for absence management, load balancing and staffing relief. In addition, the Team Leaders will maintain relationships with our Clinical Resource Team, and as appropriate, external partners and clinical staff, who provide staffing relief for THP. The Team Leaders will ensure that they remain current with Collective Bargaining Agreement (CBA) changes, policies and procedures as well as non-union agreements while providing mentorship to the WMO team. The Team Leaders will remain current with any upgrades to the workforce management software and provide ongoing education for new staff.

Leadership Responsibilities – Project Phase (April to October 2024):
- Under the guidance of the WMO Manager, lead the set-up of the WMO including: recruitment, phased starts, training, troubleshooting, and communication processes
- Develop key WMO documentation, including but not limited to policies and procedures, contact lists / call sheets, team schedule(s) / coverage
- Support the Manager, WMO and PCMs in the implementation of transition plans for moving scheduling from their area into the WMO, including resources, if applicable
- Engage with WMO Manager and Patient Care Managers, clinical leaders and executives, on plans, progress, feedback and status reporting
- Work with the WMO team to understand issues and concerns, addressing them to create a positive and productive work environment
- Implement all schedules in a standard Excel format and work in collaboration with PCM’s, ensure all schedules have been converted to the necessary format and baseline staffing compliments have been verified.
- Identify, track, and mitigate where appropriate, issues that require resolution both during the Project Phase and post-go live
- Ensure readiness of the WMO prior to go-live
- Support and advocate change management
- Become a subject matter expert / super user in the new scheduling and time keeping systems
Leadership Responsibilities (Operational & as the WMO is staffed up):
- Assist the Manager to plan, coordinate and effectively manage resources (e.g. financial, human, physical space, equipment and materials) in alignment with the Hospital’s strategic plan
- Review, analyze and apply program performance data to support decision-making and continuous quality improvement
- Support the Manager, WMO in required aspects of employee relations including human resource planning; recruitment, performance management; coaching/mentoring and professional development and escalate to the Manager any concerns
- Engage in conflict resolution in regards to situations that may arise due to inequality in staffing resources across units
- Actively support the development of the WMO team through participation in work internal and external to THP
- Support staff engagement and workforce resource planning, including retention and recruitment strategies to ensure stable workforce within the WMO
- In collaboration with PCMs attend/lead staffing calls to ensure that all clinical areas at THP have adequate staffing support, to decrease overall risk related to patient safety
- Become familiar with workforce analytics, workforce planning and resource optimization to support the WMO team in their roles and clinical leaders, senior leaders in their decision making In collaboration with the manager, design and implement initiatives that support and align with the strategic plan for the organization
- Develop and lead ongoing education for the UKG and Andgo training for new staff during General Hospital orientation or as required
- Working with the manager, develop, and report on KPI’s specific to WMO with analysis to provide opportunities for efficiency and workforce planning
- Administer the UKG Dimensions and AndGo system upgrades (in partnership with IS and the vendors), training and associated communications, working with the Team Leaders who will have accountability for these functions
- This role may require availability for on-call during weekends and after-hours if escalations are required

Job Description:
Reporting to the Manager, Workforce Management Office (WMO), the successful candidate(s) will lead a team of Scheduling Clerks accountable for staff scheduling, shift and absence management and timekeeping for nursing, allied health and clerical staff across clinical programs at all sites of Trillium Health Partners. The WMO will support a team focused approach in which the Scheduling Clerks will work with their peers and with Patient Care Managers/designate(s) to collaboratively ensure 24/7 adequate staffing coverage across clinical programs (all sites) to optimize the quality of patient care delivered. This is an opportunity to be a part of a new and exciting initiative for THP that is focused on creating OneTHP – One People Experience. The WMO will operate 24/7 to provide ongoing operational coverage for staffing relief.
The WMO is a new department within THP that will be established between Spring 2024 and Fall 2024 (the Project Phase) in a phased approach. During this period, the WMO Team Leaders will be accountable to the Manager and assist with recruitment, onboarding and training of new staff, facilitation of team building, and skill acquisition set up and maintain ongoing day to day operations of the WMO. Accountability will include the transition of clinical program scheduling development, maintenance and absence management through the WMO prior to project go-live. Upon go-live with the new scheduling and shift management system (UKG Dimensions and AndGo Systems) in Fall 2024, the Team Leaders will transition from project phase to operations along with the day to day office responsibilities.
The successful candidate(s) will excel in operational effectiveness; are committed to patient-centred design; and have a passion for leading workplace environment transformation within an interprofessional acute care setting. The Team Leaders will work in partnership with Patient Care Managers (PCMs) and delegates to understand the staffing and daily operational needs and provide oversight and guidance to WMO scheduling coordinators for absence management, load balancing and staffing relief. In addition, the Team Leaders will maintain relationships with our Clinical Resource Team, and as appropriate, external partners and clinical staff, who provide staffing relief for THP. The Team Leaders will ensure that they remain current with Collective Bargaining Agreement (CBA) changes, policies and procedures as well as non-union agreements while providing mentorship to the WMO team. Leadership Responsibilities – Project Phase (April to October 2024):
- Under the guidance of the WMO Manager, lead the set-up of the WMO including: recruitment, phased starts, training, troubleshooting, and communication processes
- Develop key WMO documentation, including but not limited to policies and procedures, contact lists / call sheets, team schedule(s) / coverage
- Support the Manager, WMO and PCMs in the implementation of transition plans for moving scheduling from their area into the WMO, including resources, if applicable
- Engage with WMO Manager and Patient Care Managers, clinical leaders and executives, on plans, progress, feedback and status reporting
- Work with the WMO team to understand issues and concerns, addressing them to create a positive and productive work environment
- Implement all schedules in a standard Excel format and work in collaboration with PCM’s, ensure all schedules have been converted to the necessary format and baseline staffing compliments have been verified.
- Identify, track, and mitigate where appropriate, issues that require resolution both during the Project Phase and post-go live
- Ensure readiness of the WMO prior to go-live

- Become a subject matter expert / super user in the new scheduling and time keeping systems
Leadership Responsibilities (Operational & as the WMO is staffed up):
- Assist the Manager to plan, coordinate and effectively manage resources (e.g. financial, human, physical space, equipment and materials) in alignment with the Hospital’s strategic plan
- Review, analyze and apply program performance data to support decision-making and continuous quality improvement
- Support the Manager, WMO in required aspects of employee relations including human resource planning; recruitment, performance management; coaching/mentoring and professional development and escalate to the Manager any concerns
- Engage in conflict resolution in regards to situations that may arise due to inequality in staffing resources across units
- Actively support the development of the WMO team through participation in work internal and external to THP
- Support staff engagement and workforce resource planning, including retention and recruitment strategies to ensure stable workforce within the WMO
- In collaboration with PCMs attend/lead staffing calls to ensure that all clinical areas at THP have adequate staffing support, to decrease overall risk related to patient safety
- Become familiar with workforce analytics, workforce planning and resource optimization to support the WMO team in their roles and clinical leaders, senior leaders in their decision making In collaboration with the manager, design and implement initiatives that support and align with the strategic plan for the organization
- Develop and lead ongoing education for the UKG and Andgo training for new staff during General Hospital orientation or as required
- Working with the manager, develop, and report on KPI’s specific to WMO with analysis to provide opportunities for efficiency and workforce planning

Job Description
The Trillium Health Partners (THP) Regional CKD Program supports one of the fastest growing regions in need of dialysis within the province. To support our community, patients and families, the program has developed a new leadership structure that ensures leadership presence and coordination of care across all sites and modalities. The Regional CKD program is responsible for pre-dialysis, chronic outpatient and home dialysis and acute dialysis care for patients within the Mississauga and South Etobicoke region.
Reporting to the Patient Care Managers (PCM) of the Regional Chronic Kidney Disease Program, The Clinical Nurse Supervisor is a fundamental component of the leadership triad working in collaboration with the PCM and Educator to ensure safe and high quality care for patients within all modalities across THP and the Regional CKD Program.
The Clinical Nurse Supervisor is responsible for the oversight of nursing, clerical staff, and renal assistants. They are instrumental in supporting the inter-professional team through daily operations and works collaboratively with the leadership team to plan, develop and implement best practices that guide patient care, workflow and efficient operations. In addition to routine daily operations, the supervisor will work with staff, physicians, patients and families to monitor patient safety, mitigate risk and implement quality improvement initiatives.
Duties and Responsibilities:
- Ensures the smooth operations of the CKD program by ensuring daily operational procedures are followed, identifying opportunities for improvement in the unit and collaborating with the Renal team to implement and sustain process changes as needed reviewing and supporting daily scheduling requirements; forecasting staffing and scheduling needs; coordination of nursing and other care team resources; ensuring equipment and supplies are adequately maintained to support efficient patient care; adherence to policies, procedures and standards of care; continuously monitoring patient flow and capacity
- Collaborates with the Education team in the creation and implementation of policies; orientation, training and staff development and investigating and resolving patient safety events
- Conducts investigation and identifies appropriate resolution for patient, family, or staff concerns
- Ensures the ongoing success of the department by: anticipating and minimizing organizational and unit based barriers; setting performance goals for achieving maximum scope of practice; demonstrating effective working relationships; and partnering with the Patient Care Managers and the Education team to ensure a collaborative work setting
- Supports and maintains the priorities of the CKD Program, as well as activities to support ORN initiatives
- Provides coaching and mentoring to members of the Renal team

Our Commitment to and Progress Towards Anti-Black Racism
THP and IBH have made a commitment to becoming a safer and more inclusive environment with a dedicated focus on dismantling anti-Black racism and white supremacy at THP. This commitment is in response to the recognition that Black people systemically experience the organization differently than their white and non-Black colleagues. THP established its Equity Secretariat Office in 2020, providing education and training on anti-Black racism, and establishing a THP Circle on Anti-Racism and Inclusion. Today, the Equity Secretariat has become the Equity, Anti-Racism and Inclusion Office, with a current staff of four whose specific focus is on dismantling anti-Black racism and white supremacy and advancing other dimensions of inequity within THP. This office supported the completion of an externally-led Anti-Black Racism Climate Review and resulting report, which includes recommendations and actions that are being integrated into THP’s operating plan. At IBH, the Anti-Black Racism Allyship Initiative (ABRAI) was established in 2020 to understand the experiences of Black staff within IBH and develop an action plan in partnership with Black staff to address anti-Black racism. The action plan has been translated into operational objectives that support the recruitment, retention, and career development of Black staff. As part of this plan, IBH has established an optional, monthly Black Excellence meeting for Black staff at IBH where the purpose is to provide a safe space where all hold power and attendees can connect, network, and support each other.
The Role
The Research Associate role within the IBH supports research studies and initiatives across the organization and within the community. Through this role, the successful candidate will support the portfolio of Dr. Lisa McCarthy, a Learning Health System Fellow at the Institute for Better Health, an Associate Professor at the University of Toronto’s Leslie Dan Faculty of Pharmacy and Department of Family and Community Medicine, and co-lead of the pan-Canadian deprescribing.org team. This role will support a series of research initiatives focused on optimizing the appropriate use of medications and improving care for people across the lifespan, particularly older adults and those living in long-term care homes, through deprescribing and optimizing care transitions.
Within this dynamic portfolio, there are several central topics of interest, including: designing, implementing, and using mixed-methods to evaluate interventions that support optimal medication use across health systems; conducting knowledge syntheses; using a behavioural science lens to understand success and failure of healthcare systems and innovations; and co-designing knowledge mobilization approaches to support implementation of evidence-based clinical practice guidelines and other resources to support deprescribing as part of primary and long-term care settings. Engagement with stakeholders, including patients and caregivers, is a central component of the role.
The successful candidate will have the opportunity to work in a multivariate stakeholder environment both independently and as part of collaborative, multi-disciplinary focused research groups supporting various projects. This role facilitates the responsible conduct of research through support of research studies within THP. This support includes coordinating project logistics and timelines, communicating with various internal and external stakeholders, and leading data collection and analyses of various research studies. This is a unique opportunity to be exposed to the hospital setting, support innovative health research, and play a pivotal role in enabling and mobilizing health research within and beyond the walls of Trillium Health Partners.

- Lead the preparation and submission of research ethics, study coordination, analysis and dissemination of results and the organization and coordination of a major portion of the research activities under the supervision of the principal investigator

POSITION SUMMARY

We are building a new kind of healthcare for the highest growth hospital in Ontario, leveraging a newly implemented health information system. This is a great opportunity for someone who is interested in joining a group that is evolving and to be able to use hospital data to help clinical leaders to drive decisions and tackle complex business questions. In this role you will be reporting to the Manager of Decision Support and will be responsible for:
- Engage in program operations, to provide integrated analysis and insights on program performance together with Finance, HR, Strategy Management Office, Business Performance Office, Quality to support clinical operations and external submissions
- Play a key role in the development BI tools (dashboards, reports) in support of hospital operations.
- Educate various stakeholders on funding methodologies, and other clinical methodologies, including recommendations to enhance quality of care and optimize funding performance
- Project management to support the collection and reporting of new structural, process, and outcome indicators to internal and external stakeholders working closely with Business Intelligence Advisors.
- Knowledge transfer/exchange strategies with internal and external leaders regarding interpretation of reports and utilization of various business intelligence technologies (e.g. Power BI, SSRS, Excel etc.) to enhance decision making.
- Active engagement and coordination in the annual hospital planning cycle including financial, clinical, and statistical budgeting and forecasting models, strategic business case development, physician impact analyses, patient flow modeling, bed impact analyses, etc.
- Support evidence informed decisions through a range of analytics from descriptive to diagnostic, predictive and prescriptive
- Provide insights on patient experience results
- Identify risk and opportunities of funding in relation to clinical volumes
- Leverage case costing and other benchmarking data to inform efficiencies
- Forecast, track and measure key business metrics including but not limited to length of stay index, readmissions, clinic visits, to support data-driven decisions.
KEY QUALIFICATION
- University Degree in Statistics, Mathematics, Engineering or a related discipline (Graduate degree is preferred)
- Experience in Business Intelligence Tools (Power BI, Tableau)
- Experience with clinical datasets (DAD, NACRS, CCRS, NRS, OHMRS, etc.) submitted to the Canadian Institute for Health Information and case costing data.
- Advanced proficiency in SQL and Excel
- Experience using IDS, Intellihealth, CIHI Portal and iPort for benchmarking purposes.
- Knowledge and understanding Patient Based funding.
- Demonstrated proficiency in quantitative and statistical analysis
- Demonstrated effective communication (written and verbal) and interpersonal skills with all levels of staff including physicians, patient service leaders, Ministry Representatives and various other internal/external client groups

- Working knowledge of MOHLTC Reporting and MIS Guidelines.

The Role
Our successful candidate will report to the Associate Vice President, Philanthropy. They’ll inherit and lead a high-performing team of 4; a Manager, a Senior Development Officer, Development Officer and a Coordinator Our successful candidate will be seasoned team leader and will focus on Major, Corporate and third party event growth, increased donor stewardship, pipeline management and team mentorship.
PRIMARY RESPONSIBILITIES
- Revenue Generation & Senior Level Volunteer Support
- Provide leadership to a team of fundraisers and, supporting with cultivation and solicitation strategies to advance our mission and achieve ambitious targets.
- Build and manage a portfolio of major gift donors with the capacity to make a gift of $250K, $500, or $1M+ toward a team goal
- Lead & support meetings and presentations with prospective and current donors
- Collaborate with team and develop donor strategies to engage and inspire community members towards gift commitments
- Maintain frequent and appropriate communication with the donors through in-person meetings, emails, phone, zoom, etc.
- Advise on named gift opportunities and donor recognition plans in order to maximize donor engagement
- Support with Board and Cabinet Member solicitations
- Work with senior level volunteers from the community and internally from the hospital to identify prospective donors, secure meetings, close and steward gifts
- Actively seek out new prospects interested in advancing the mission and success of THPF
- Engage and coach volunteers in a meaningful way to generate enthusiasm and successful outcomes
- Strategic oversight of Corporate and (3P) Community Fundraising
- Lead the team responsible that executes and leads the strategy of our corporate and third party philanthropy programs
- Provide leadership and accountability for major gift and third party fundraising programs
- Solicit and steward corporate gifts through major gifts and sponsorship channels toward the achievement of a corporation’s greatest impact as a donor
- People Management
- Provide support and guidance to 3 Direct Reports (Manager, Senior Development Officer and Coordinator), and indirectly to a Development Officer.
- As a leader within the broader Foundation team, act as a collaborative resource, advisor and mentor to build fundraising capacity and help instil best fundraising practices
- Support with the retention, development and performance of the team
- Provide guidance to team regarding engagement with diverse cultural communities
- Offer support and training to colleagues on Raisers Edge database
- Hospital Relationships
- First point of contact for many hospital colleagues, providing assistance and triaging requests on a case by case basis
- Work with hospital partners t to identify and engage corporate hospital vendors to advance meaningful, philanthropic partnerships with THP
QUALIFICATIONS
- 8-10 years of major gift experience in a sophisticated philanthropic institution, ideally within a health care system.
- Demonstrated experience cultivating, soliciting, and stewarding individual, corporate, and foundation gifts at the $250K - $1M+ level.
- Demonstrated experience and success managing, motivating, and mentoring development professionals bringing out the best in teams and individuals.
- University or College degree
- Certified Fund Raising Executive (CFRE) designation or working toward is considered an asset.
- Comprehension of donor management systems (Raiser’s Edge or RENXT).
Work Environment
- Willingness to work flexible hours and to tight deadlines as needed —some after-hours work may be required.
