Position Summary:
Trillium Health Partners (THP) has a 39 bed post-acute medically Complex Continuing Care (CCC) unit at our Queensway Health Centre (QHC) location and recruiting an individual who is highly skilled, agile and adaptable with advanced critical thinking and interprofessional skills. As a member of the CCC, QHC Interprofessional team you will have the opportunity to provide care to the medically complex continuing care patient population.
Responsibilities:
- Working as a member of an Interprofessional team to provide quality care to the medically complex patient population
- Responsible for the provision of nursing care, teaching and coaching to patients and their families
- Responsible and accountable for the provision of nursing care in accordance with the standards of nursing practice and THP Best Practice ProtocolsKey
Qualifications:
- A current annual certificate of competency with the College of Nurses of Ontario
- Medication Certificate and experience with administration of medication within the last year required
- Previous experience in Complex continuing care, Rehab nursing concepts, Gerontology and treatment approaches required
- Experience with Tracheostomy care
- Experience with patient centred and partnership care models
- Must have effective interpersonal, communication, organizational and problem solving skills
- Reliable and flexible individual who is willing to work collaboratively within an interdisciplinary team environment
- Positive attendance and work record essential

Job Description:
The successful candidate will be responsible for receiving, verification, ordering, counting, delivery and the replenishment for all supplies for central stores and for the stockless program (JIT), Candidates will also be responsible for computer entry, par analysis and min/max updates and changes.
Qualifications:
- Grade 12
- Familiarity with Meditech Materials Management Module and proficiency with computers and/or data entry an asset
- Familiarity with medical/surgical and business products an asset
- Ability to perform accurate inventory counts
- Demonstrated oral and written communication skills

- As an active member of a multidisciplinary team, the dual trained OTA/PTA provides client-centered clinical care for assigned patients under the direction and supervision of a Physiotherapist and Occupational Therapist.

Job Description:
The successful candidate will deliver and pick up supplies and equipment, assist with loading, unloading, stocking, order picking, and other related duties as assigned. Position may require availability 7 days a week on a rotational basis and shift times can be subject to change.
Qualifications:
- Grade 12 or equivalent
- Must be able to read, write and speak English
- Be physically capable of performing the essential duties of the job
- Lifting up to 45lbs
- Possess excellent interpersonal and communications skills
- Preference will be given to applicants with recent related experience

4J Surgical Spine is a 16 bed unit that provides acute care to patients before and after spinal surgery.
We are the regional service for spine specific interventions, serving most commonly Mississauga, Etobicoke, Brampton, Oakville, Georgetown, Milton and Orangeville.
4J Surgical Spine Unit is supported by an inter-disciplinary team of RNs and RPNs, CQCL, Charge Nurses, Physician Assistants, Physiotherapists, Occupational Therapists, OTA/PTA, Speech Language Pathologists, Discharge Planners, Dieticians, Pharmacists, 7 Neurosurgeons and 2 Orthopedic surgeons. The Inpatient Medicine & ICU Program at THP consists of a total of more than 350 beds across the CVH & MH and is staffed by large interprofessional teams who support the care of complex medicine patients within the Mississauga-Halton LHIN. Our in-patient units offer first-rate care in an innovative environment supporting the physical, mental, emotional and spiritual needs of our patients. Reporting to a Clinical Manager, the ideal candidate will be focused on providing excellent patient centered care in a dynamic environment where he/she will be given the opportunity to deliver care with compassion, excellence and courage.
Required Qualifications:
- Graduate of a recognized college or school of nursing
- Registered in good standing with the College of Nurses of Ontario (CNO) or currently meet the eligibility criteria to register in good standing in the Temporary Class or General Class with the CNO.
- Able to function to the full scope of practice as per CNO
- Basic Life Support certification
- If working in an area with telemetry, successful candidates responsible for obtaining formal certification within six months of hire
- Demonstrated knowledge, experience and the ability to care for patients with the following care needs: initiation and maintenance of IV therapy, administration of medication above and below the drip chamber, blood and blood product administration, patient controlled analgesia, Enteral feeds, TPN, transfusion of blood products, central venous access devices CVADS (PICCs), blood glucose monitoring, naso-gastric tubes, wound management skills, routine infection control practices, normal and abnormal blood values, oxygen therapy, tracheostomies, and chest tubes.

Preferred Qualifications:
- Baccalaureate degree in Nursing preferred
- Minimum of two years acute care medical/surgical experience
- Completion of a physical assessment course
